At DaVita, we see each patient as a unique individual with specific needs that go beyond the physical process of the disease that brought them into our care.
Associated with the disease process are psychosocial, family, and socioeconomic challenges, which we are sensitive to and capable of helping each individual and family overcome.
Social service professionals are available at each DaVita Unit to support patients and their families in these challenges, which are varied and change over time, coordinating with the rest of the clinical team and developing the best plan for the well-being and health of patients and their families.
Identifying the socioeconomic and psychosocial needs and potential of each patient and family, providing emotional support, liaising with other health and social services and other community resources, facilitating the obtaining of documents, scheduling dialysis holidays, mediating problems associated with transportation services, and promoting the voice of patients and families in the health-illness process are some of the important missions that the social worker in each DaVita unit carries out.
